These forms are samples only, demonstrating a scope of survey and should not be filled out, as all reporting is logged electronically with exceptions of the "Part-D LoD" and "Hatch Cover UT Report Form" that can be filled and attached to the formal report.

The detailed instructions for condition surveys will continue to be provided via instruction letter, taking into account the relevant details of the survey on a case-by-case basis
